Medication review
We reconcile every prescription, supplement, and over-the-counter — and look for interactions you didn't know about.
Cognitive screening
Standard memory and cognitive checks, with referral to specialty assessment when warranted.
Falls prevention
Balance assessment, home-hazard review, and physiotherapy referrals.
Care planning
Conversations about goals of care, advance directives, and family caregiver support.
How to prepare.
- →Bring all medications in their original bottles.
- →Bring a family member or caregiver if you'd like a second set of ears.
- →Write down recent hospital visits, specialist appointments, and any new symptoms.
- →Bring eyeglasses and hearing aids if you use them.
After your visit.
Pharmacy receives any updated prescriptions.
Specialist referrals are faxed; you'll hear from the specialist's office directly.
We recommend a follow-up to review chronic-condition management.

Do you do home visits?+
We do not currently offer home visits. For housebound patients, we recommend the Manitoba Home Care program.
Can my family caregiver come into the room?+
Absolutely — and we encourage it. Bring whoever helps you keep track of your health.
